Purpose of Agreement

This agreement outlines the terms and conditions under which hypnotherapy services are provided by the Hypnotherapist, Chris Parker, to the Client. By signing, the Client acknowledges understanding and acceptance of these terms.

1. Confidentiality

All sessions and personal information shared are treated with strict confidentiality. No information will be disclosed without written consent, except in cases required by law (e.g., suspected abuse, threats of harm to self or others, or court orders).

2. Payment of Services

All payments are due at the time of service unless otherwise arranged in writing.

• Standard session fee: - $160 - ($95 if client is going through economic difficulty)

• Accepted forms of payment: Cash, card, Venmo, Unpaid balances may result in suspension of future sessions until resolved.

3. Cancellation Policy

Cancellations require at least 24 hours' notice.

• Cancellations made with less notice may incur 50% of session cost.

• Missed appointments without notice will be charged in full. Repeated cancellations may result in termination of services.

4. Scope of Practice and Limitations

The Therapist does not diagnose mental or physical disorders, prescribe medication, or provide medical treatment. Hypnotherapy is a complementary wellness approach, not a substitute for medical or psychological care. Clients currently under medical or psychiatric care are encouraged to consult their provider before beginning hypnotherapy.

5. Issues Not Addressed

The Therapist does not work with:

• Active substance abuse or addiction recovery without concurrent medical supervision

• Severe psychiatric disorders requiring clinical or hospital care (e.g., schizophrenia, bipolar disorder)

• Untreated trauma that requires specialized psychological intervention

• Any situation outside the Therapist’s training or ethical boundaries

6. Disclaimer of Medical or Psychiatric Treatment

Hypnotherapy services do not replace professional medical, psychological, or psychiatric treatment. The Hypnotherapist makes no medical or outcome guarantees. Clients remain responsible for their own health decisions and treatment needs.

7. Intense Personal Experience Disclaimer

Hypnotherapy can bring up strong emotions, memories, or insights. While every effort is made to provide a supportive environment, the process may occasionally feel emotionally intense. The Client agrees to take personal responsibility for self-care and to communicate any distress during sessions.

8. Group Session Disclaimer

In group hypnotherapy or educational settings, confidentiality cannot be guaranteed. Participants are encouraged to respect each other’s privacy, but responsibility for discretion rests with each individual.

9. Right to Work With or Not Work With Clients

The Hypnotherapist reserves the right to accept or decline clients based on professional judgment, suitability of goals, compatibility, or ethical considerations. Clients also hold the right to end their participation at any time.

10. Acknowledgment and Agreement

By signing below, the Client acknowledges understanding and agreement with the terms and conditions outlined in this Hypnotherapy Service Agreement.
